About the Role
We are seeking a driven and relationship-focused Community Liaison to join our team in Coeur d’Alene.
This role is ideal for someone who thrives in a fast-paced, people-centered environment and is
passionate about building strong partnerships within the healthcare community.
Key Responsibilities
• Build and maintain strong relationships with referral sources, including physicians, hospitals, skilled
nursing facilities, and assisted living communities
• Promote and educate partners on home care services, clearly communicating value and identifying
client needs
• Serve as the primary liaison between referral sources, clients, and internal teams to coordinate care
and ensure a smooth start of services
• Maintain ongoing communication with partners to support satisfaction, address concerns, and
strengthen long-term relationships
• Represent the company in the community through outreach efforts and in-home visits with
prospective clients
• Monitor market feedback and referral activity, providing insights and recommendations to improve
growth and marketing strategies
• Stay informed on company policies, procedures, and industry trends; participate in team meetings
and support overall team goals
Qualifications
• Sales or business development experience preferred, but not required
• Strong interpersonal and relationship-building skills
• Excellent communication and organizational abilities
• Self-motivated with the ability to work independently and as part of a
team
• Healthcare experience is a plus, but not required
